Building a Healthy Mississippi

The application should be completed by the School Health Council. The school Principal and the Superintendent must attest to the best of their knowledge the application questions have been answered accurately and completely.

To submit an application, the school must create an account and complete the online 2018 Healthy School Awards application.

Completing the Application

Please provide a response to each question. Do not answer a question with a reference to another response within the application (e.g. see response to question 1.1).

Questions requesting documentation refer to items such as photos, sample curricula, schedules, staff roster, news releases, meeting minutes, flyers, brochures, lesson plans, webpage screen shots, written statements, etc. Schools must submit required documentation to receive credit for response.

Application Submission

Deadline: The deadline for submitting the 2018 applications is Friday, October 27, 2017.

Applications must be submitted online at this website.

In addition to completing this application, attach documentation, such as: creative menus, signage, newspaper articles and pictures to enhance the application.

Once the application is submitted, it is considered final and no changes can be made.

Applications must be submitted no later than 11:59 p.m. Central Time, Friday, October 27, 2017. Applications submitted after that time/date will not be accepted.

You will receive a reply to confirm the Blue Cross & Blue Shield of Mississippi Foundation received the application.
